All volunteers must complete a Medical Emergency Form. Click Here to download the form. The form is fillable so you can save the completed form to your device and email it to joe@forestbaptistchurch.org.

If you are a member of FBC and need financial assistance to participate on a mission trip, you may qualify for a partial scholarship. Click Here to download a Missions Scholarship Fund Application. Please return the completed form Joe Veres, Minister of Missions, as directed on the form.

Families pursuing adoption (either domestic or international) can apply for the Forest Baptist Church Adoption Scholarship. Click Here to download the form. The form is fillable so you can save the completed form to your device but be sure to print the final version so that the last page can be signed. A scanned copy can be emailed to joe@forestbaptistchurch.org or hard copies can be returned directly to Joe Veres, Minister of Missions.

All events (both ministry and personal) must first be approved by our Church Administrator and all guidelines must be followed.

All prospective members must complete their testimony for membership. If you plan to join by baptism, please download and complete this version. If you plan to join by statement or letter, please download and complete this version.

Members of FBC (as well as members of the Forest community) who have short-term financial needs can make benevolence requests of our deacon ministry. Please download and complete the form and return it, in person, to the church office during normal office hours. If you have any questions, please contact the church office.
